Why a Government Forms Database? 

In the fast-paced world of production, staying compliant with government regulations is crucial. Until now, essential forms have been strewn across the internet, tucked away in different departments and sub-menus.

Our Government Forms Database addresses this clutter by providing a centralized, easily accessible repository of essential forms. This tool aims to save you time and ensure compliance across various federal, state, and local jurisdictions whether you’re shooting in one location or across multiple states!

What does our Government Forms Database include?

Our database offers a wide array of federal, state, and local forms critical to the entertainment industry. Some of the most important ones include:

Federal forms

  • W-2: A tax form provided by employers to employees, summarizing their annual earnings and the amount of taxes withheld for reporting to the IRS.

  • W-4: A tax form completed by employees to inform their employers of the amount of federal income tax to withhold from their paychecks. It factors in personal allowances, dependents, and any additional income or deductions, guiding employers in accurately deducting federal taxes from employees' earnings.

  • W-9: A tax form used by individuals and entities to provide their taxpayer identification number (TIN) to entities that will pay them income, ensuring the proper reporting of earnings for tax purposes. It is commonly requested by businesses from freelancers, independent contractors, and other non-employees to accurately report payments to the IRS.

  • 1099-MISC: A tax form used by businesses to report various types of income paid to non-employees, such as independent contractors and freelancers, to both the recipient and the IRS.

New York forms

  • New York City Parks Film Shoot Request Form: A form requesting permission to film in one of New York City’s Parks.

  • NYC Drone Permit: Application for a permit to fly a drone in NYC. Members of the public may apply for permits authorizing take-offs and landings of unmanned aircraft (UA), or drones, throughout New York City. A permit is required in order to take-off or land any FAA-registered, FAA-compliant UA lawfully within the City.

  • Sample Resident Notification Letter: A sample letter to distribute to residents affected by production in New York City.
